General Instructions for All Rebates:
1. Complete all five sections of the rebate request form
2.Attach a photocopy of the receipt of purchase for the copy of Peachtree Accounting 2005 product for which you are requesting a rebate. Please circle purchase date and amount paid.
3.Cut the Peachtree Accounting 2005 product UPC barcode from the bottom of the Peachtree Accounting package you purchased and include it with this form. The original UPC bar code is required.
4. Include Proof of Ownership from previous Peachtree or competitor's product.
5. Make a copy of you complete submission for your records.
6. Mail your completed rebate request form and all accompanying items to the appropriate address below (must be postmarked by December 31, 2005).
